Complex Patient Care in Community and Emergency Room (ER)
Discharge rounds and community rounds take place using a team based care approach with allied health, outreach workers, and community paramedics every week to discuss complex cases and potential troublesome patients in the community.
[We] have a mechanism for allied health. In this room [we] have discharge rounds and making those plans, and then have community rounds [with] social work, occupational therapy, physio therapy, home health, community paramedicine, [and an] outreach worker every Tuesday to discuss how to address complex case and discuss patients in community that people are worried about. Helps [us] be familiar with complex patients that present often so that if you are admitting them or they show up in [the Emergency Room] ER you know a lot more about them.
